File a Claim

We make the claims process as simple and transparent as possible. To begin, fill out the form below with your basic information.

 

A designated ATI representative will reach out within 2 business days to complete the claim submission.​ For Trauma Insurance policies, you’ll need the following information for claim approval:

  • A police or incident report

  • Proof of employment or membership

  • The date the incident occurred
